Option 1: Stop order authorization

I hereby authorize IBSA to deduct from my salary/commission account, the subscription fee as determined by the Trade Union from time to time.

Option 2: : Debit order authorization

I herewith give permission that the Employer may share my personal information with IBSA. All Personal Information that we collect on this form will be processed, and held in accordance with the provisions of the Protection of Personal Information Act, your rights thereunder as well as our Protection of Personal Information Policy. You may request a copy of our Protection of Personal Information Policy from Daphne, at ibsa@ibsa.org.za. By not providing us with the Personal Information as requested on this form, we will not be able to contact you with news, updates and offers. :
